Office Policies


  1. Billing and insurance information MUST be provided prior to your appointment day. Due to in-office time constraints, we will be unable to verify benefits on the day of your appointment. If insurance information is not provided, please expect to pay out of pocket and self-submit your claim.

  2. Our office is located inside Costco but we are independently owned, and hence are in network with different insurances than Costco Optical, despite what you may see on your insurance plan website. The only way to know for certain if we are an in-network provider is to send us your insurance information 48 hours prior to your appt for us to verify your benefits.

  3. All patients are required to fill out and submit the intake form online prior to the day of your appointment.

  4. If you are more than 10 minutes late for your appointment, we may have to reschedule.

  5. All no-shows and cancellations with less than 24 hrs notice are subject to a $50 fee.
